Lack of diagnostic gold standard, children need to be vigilant when they have fever!

In the past month, six children with Kawasaki disease were treated in the pediatric department of a hospital in Xi ‘an, four of them were in serious condition, and one child also developed Kawasaki disease shock syndrome. What exactly is this disease? What’s the performance? Many parents said that they have never heard of it and have never seen it.

Let’s get to know Kawasaki disease through a case.

A boy, 1 year old and 5 months old, was admitted to the hospital because of "fever for 6 days".

Six days ago, the child had no obvious cause of fever, with a heat peak of 39.7℃, accompanied by runny nose, nasal obstruction, cough, shortness of breath, wheezing, convulsions, vomiting and diarrhea. Fever did not improve after 4 days of oral administration of cefaclor dry suspension in outpatient department.

Physical examination: conscious, ruddy complexion, palpable swollen lymph nodes on both sides of the neck, about 2cm*1cm in size, no tenderness, no redness and swelling, but movable, scattered red rash on the trunk skin, fading after pressure, conjunctival congestion, no purulent secretion, dry red lips, no chapped lips, red bayberry tongue, pharyngeal congestion, swollen tonsils on both sides, no purulent secretion. Flushing and swelling can be seen in the palm and sole, and no peeling is found at the toe end. Scar without redness, perianal flushing, peeling.

The child has no history of drug or food allergy, and is vaccinated on time.

Auxiliary examination: blood routine: white blood cell 17.06×109/L, neutrophil 72%, hemoglobin 105g/L, platelet 310×109/L, C-reactive protein 57 mg/L; ESR 80 mm/h; There is no abnormality in liver function; Anti-streptolysin O, seven respiratory virus antigens, mycoplasma pneumoniae PCR(-), normal immunoglobulin, blood culture (-); The right coronary artery diameter was 1.9mm and the left coronary artery diameter was 2.0mm by echocardiography.

Diagnosis: Kawasaki disease.

What is Kawasaki disease?

Kawasaki disease, also known as cutaneous mucosal lymph node syndrome, can’t be prevented, and the cause is still unclear. It is generally believed that Kawasaki disease is an acute systemic immune vasculitis triggered by infectious factors, with fever, jealousy, lip redness, rash and cervical lymph node enlargement as the main clinical manifestations. Cardiovascular injury, especially coronary artery disease, is an important complication of Kawasaki disease, which can lead to myocardial ischemia, myocardial infarction and even death, seriously endangering the health and life of children. Kawasaki disease mainly occurs in children under 5 years old, and can occur all year round, especially in East Asia such as China and Japan. The data published in recent years show that there are more than 100 new cases of Kawasaki disease per 100,000 children aged 0-4 every year.

What are the manifestations of Kawasaki disease?

Main clinical manifestations

1. Fever: It is often repeated fever, and the heat peak can reach 39-40℃, and antibiotic treatment is ineffective.

2. Changes of extremities: In the acute stage, the palms and soles flush and hard edema appear. At 2-3 weeks, desquamation (membranous desquamation) of fingers and toes starts from the nail week and can extend to the palms and soles of feet. The course of the disease lasts for 1-2 months, and deep transverse groove (Beau’s line) or nail removal may appear on the nail.

3. Rash or redness at the BCG vaccination site: Rash usually appears within 5 days after fever, and there are common diffuse maculopapular rash, scarlet fever-like rash and erythema multiforme-like rash. Rash is usually widely distributed, mainly involving trunk and limbs. The aggravation of rash in groin and early desquamation, perianal flushing and desquamation are the characteristics of Kawasaki disease. Scar swelling is a relatively specific early manifestation of Kawasaki disease.

4. Bilateral bulbar conjunctival congestion: Children may have bilateral bulbar conjunctival non-exudative congestion shortly after fever.

5. Oral and oral changes: including red lips, dryness, chapped lips, peeling and bleeding, strawberry tongue, and diffuse congestion of oropharyngeal mucosa.

6. Non-suppurative enlargement of cervical lymph nodes: usually unilateral, with a diameter ≥1.5cm.

Auxiliary inspection

1. Laboratory inspection:

① Blood routine showed an increase in white blood cell count, mainly neutrophils; Decreased hemoglobin; The increase of platelet count usually appears in the second week of the course of disease, reaches its peak in the third week, and returns to normal in 4-6 weeks; A small number of children can have a decrease in platelet count, which indicates that the condition is serious.

② Urine routine showed leukocytosis, but urine culture was negative.

③C-reactive protein and serum amyloid A(SAA) increased, and erythrocyte sedimentation rate (ESR) increased.

④ Transaminase increased, total bilirubin increased, creatine muscle enzyme and myocardial isoenzyme increased, albumin and blood sodium decreased.

⑤ Serum inflammatory factors such as interleukin (IL)-6, IL-1 and tumor necrosis factor α (TNF α) are increased.

⑥ Plasma brain natriuretic peptide (BNP) or N-terminal pro-brain natriuretic peptide (NT?proBNP) increased, procalcitonin increased slightly and moderately, and serum ferritin and plasma dimer increased.

2. Echocardiography: Echocardiography should be performed as early as possible in the clinical diagnosis of Kawasaki disease to find out whether there is coronary artery damage, such as coronary artery dilatation or coronary artery aneurysm formation, and whether there is thrombosis should be paid attention to.

How to diagnose Kawasaki disease?

Kawasaki disease is a clinical syndrome, the diagnosis mainly depends on clinical manifestations, combined with laboratory examination and exclusion of other diseases. Kawasaki disease includes complete Kawasaki disease (CKD) and incomplete Kawasaki disease (IKD).

1.CKD: fever with at least 4 main clinical features of the following 5 items:

(1) bilateral bulbar conjunctival congestion;

(2) Oral and oral changes: dry and red lips, strawberry tongue and diffuse congestion of oropharynx mucosa;

(3) Rash, including red and swollen scar;

(4) Changes of extremities: redness and swelling of hands and feet in acute stage, and desquamation around nails in recovery stage;

(5) Non-suppurative cervical lymphadenopathy.

2.IKD: Children with fever ≥5d but with less than 4 main clinical features are assessed as IKD according to the flow chart in Figure 7.

What conditions need to be considered for Kawasaki disease?

According to the 2017 American Heart Association guidelines, Kawasaki disease should be considered in the following situations:

(1) long-term fever and irritability of infants younger than 6 months;

② Infants have long-term fever and unexplained aseptic meningitis;

③ unexplained long-term fever or shock with negative blood culture in infants or children;

④ Infants or children have long-term fever and cervical lymphadenitis, and have no response to antibiotic treatment;

⑤ Infants or children have long-term fever, retropharyngeal or parapharyngeal cellulitis, and have no response to antibiotic treatment.

Kawasaki disease needs to be differentiated from which diseases.

1. Scarlet fever: Kawasaki disease can also have a scarlet fever-like rash, but the simple scarlet fever infection is mostly effective for antibiotic treatment. After antibiotic treatment, the symptoms are improved and the inflammatory index is obviously reduced.

2. Other virus infections (such as adenovirus, enterovirus): In virus-infected blood, the routine white blood cell count is not high or low, but lymphocytes are increased, C-reactive protein, SAA and ESR are often not significantly increased, and the virus antibody in blood can be significantly increased.

3. Septic shock syndrome caused by staphylococci and streptococcal toxins: It needs to be differentiated from Kawasaki disease shock syndrome, especially when antibiotic treatment is ineffective, closely observe the other five characteristic clinical manifestations of Kawasaki disease except fever, and conduct echocardiography in time.

4. Drug hypersensitivity such as Stevens Johnson syndrome: There is a history of using sensitive drugs, and the mucosal manifestations are usually more serious. Moreover, the ocular symptoms are not only conjunctival congestion, but also catarrhal, suppurative and pseudomembranous conjunctivitis, which can leave eye complications.

How to treat Kawasaki disease?

Although Kawasaki disease cannot be prevented, it can be treated. Once it is diagnosed, it should be treated as soon as possible:

① High-dose intravenous immunoglobulin (IVIG): 2g/kg, the infusion time is usually 10-12h, and the dosage of 1g/kg per day can be used for children with heavy weight (such as > 20kg) for two days.

② Anti-inflammatory effect of aspirin: 30-50mg/(kg d), taken orally for three times. If the diagnosis of Kawasaki disease is delayed for more than 10 days or even longer, it is still recommended to give the above treatment as long as there are clinical symptoms and/or inflammatory indicators are still abnormal; If the clinical symptoms have subsided, the inflammatory indicators have returned to normal, and echocardiography shows no coronary artery lesions, only subsequent antiplatelet therapy and follow-up can be given.

③ Anti-platelet effect of aspirin: After 48-72h, the inflammatory indexes (white blood cell count and C-reactive protein) returned to normal, and aspirin was reduced to 3-5mg/kg. For children with no coronary artery disease or mild coronary artery dilatation in acute phase but returning to normal within 30 days, aspirin will continue to be used for 2-3 months.

Difficulties in diagnosis and treatment of Kawasaki disease in children

1. Kawasaki disease mainly depends on the clinical features, combined with the manifestations of systemic multi-system vasculitis and laboratory examination for clinical diagnosis, which lacks the gold standard and needs to exclude other diseases. The diagnosis of CKD is usually not difficult, but for the diagnosis of IKD and severe Kawasaki disease, clinicians need to be highly vigilant, combine laboratory indicators with echocardiography to make a comprehensive judgment, and sometimes even need dynamic observation during the course of the disease to make a diagnosis.

2. The main clinical features of Kawasaki disease rarely appear at a single point in time, and some clinical features will fade within a few days, so it is necessary to ask the medical history and physical examination carefully. Kawasaki disease is a self-limited disease, and the fever can stop automatically. Clinicians may ignore the main clinical manifestations of Kawasaki disease because of the cessation of fever, thus delaying the diagnosis and failing to improve echocardiography in time to evaluate whether there is coronary artery damage.

3. Kawasaki disease is systemic vasculitis, with multiple system injuries. The first symptom can be some specific organ injuries, and sometimes it is misdiagnosed as other diseases, such as urinary tract infection, suppurative lymphadenitis or mumps, acute abdomen, hepatitis and arthritis.

Summary:

Simple fever in the early stage of Kawasaki disease may be mistaken for a cold. Parents should see a doctor again in time for children with repeated fever with rash, jealousy and red lips. For children with fever for more than 5 days and poor antibiotic treatment effect, clinicians should consider the possibility of Kawasaki disease, ask for medical history and physical examination in detail, dynamically follow up relevant auxiliary examinations, and improve echocardiography in time. Children treated with IVIG rarely die of Kawasaki disease, and most of them have a good prognosis. The long-term complications are mainly related to the degree of coronary artery involvement. Take medicine according to the doctor’s advice after discharge, follow up regularly for fever and review echocardiography to evaluate the heart involvement.

  At that time, some new car-making enterprises in the new energy vehicle market began to emerge, and traditional car companies such as Geely, BAIC, Jianghuai and Changan were all testing new energy sources. Why does Dong Mingzhu prefer Yinlong, which was established only eight years ago?

  Yinlong once made a difference in the new energy bus track. In 2016, Yinlong’s business was mainly the manufacture of new energy buses and lithium batteries. On the track of commercial vehicles, there is a blessing for the development prospect of new energy vehicles, and Dong Mingzhu is firmly optimistic about the future value of Yinlong. In the next two years, Yinlong proved Dong Mingzhu’s vision with sales.   But the blow followed. In January 2018, Yinlong was repeatedly exposed to arrears in supplier payment, and there was a problem in the capital chain. By November 2018, Yinlong exposed its "dirty laundry" and publicly disclosed that its major shareholder Yinlong Group, actual controller Wei Yincang and former executive Sun Guohua were suspected of encroaching on the company’s interests by over 1 billion yuan. In 2019, the sales volume of Yinlong New Energy Bus declined in a cliff-like manner, with the annual sales volume of only 2,708 vehicles, down 62.76% year-on-year. The following year, Yinlong began to lose money continuously, and even began to be auctioned by Zhuhai Intermediate People’s Court. By the end of 2020, Yinlong’s liabilities had exceeded 22.5 billion yuan.

  Although "Yinlong became a hole", Dong Mingzhu still insisted on buying Yinlong. In August, 2021, Gree acquired 30.47% equity of Yinlong New Energy for 1.83 billion yuan through public bidding through judicial auction. In addition, 17.46% equity previously held by Dong Mingzhu was entrusted to Gree, and Yinlong officially became a holding subsidiary of Gree Electric, and was later renamed Gree Titanium New Energy (hereinafter referred to as "Gree Titanium").

  Gree’s blood transfusion also failed to rejuvenate the sales of Gree Titanium. In 2022, Gree Titanium sold 1,789 buses, down 11.65% year-on-year. In the same year, the sales of new energy buses in (), Suzhou Jinlong and () ranked in the top three with 11,515, 5,306 and 5,032 respectively.

  Dong Mingzhu said in March 2024 that she believed Gree Titanium New Energy could turn losses into profits that year. According to Gree’s mid-year report in 2024, as of June 30, 2024, Gree Titanium New Energy had a total debt of 24.786 billion yuan and a net loss of 1.905 billion yuan. However, according to Gree Electric’s financial report, the net profit of home appliance business can still cover the loss of car-making business, but the commercial vehicle track, lithium titanate battery and hydrogen fuel power battery technology bet by Gree Titanium have not yet entered the harvest period.

  The road of building cars is a new way out for Gree by Dong Mingzhu. Gree has been seeking diversified development since 2012, which is also a strategy that Dong Mingzhu vigorously promoted when he took over Gree.

  She is keenly aware that the era of high growth of household air conditioners is gradually coming to an end. According to the data, from 2010 to 2015, the year-on-year growth rate of Gree’s operating income gradually declined. Dong Mingzhu’s task is to lead Gree to find a new growth curve. To this end, she has tried mobile phones, small household appliances, new energy, medical care and even prepared vegetable tracks.

  Dong Mingzhu started with car air conditioning business. After personally taking a stake in Yinlong, Dong Mingzhu said that Gree Electric had been studying automobile air conditioning three or four years ago and had fully mastered the technology of this product. In 2017-2018, Yinlong, which was in full swing, made Dong Mingzhu see the hope of Gree’s transformation, and also planned to enter the new energy car track vigorously in the future.

  Dong Mingzhu attaches great importance to the layout of battery business. Except for the new energy bus business, Gree Titanium has always chosen lithium titanate as its own technical route. In the field of new energy vehicles, lithium carbonate and its derivative battery products, Ferrous lithium phosphate and ternary lithium battery, are widely used at present. Dong Mingzhu’s "safer" lithium titanate battery has low energy density, so its application space in domestic transportation field is relatively small.   However, Gree Titanium, which insists on the lithium titanate battery track, also shows signs of compromising to the market.   In terms of financing, Gree Titanium has no other sources of funds except the support of Gree Electric and Dong Mingzhu. Netease Cloud Music’s road to listing is a bit difficult.

Can Netease Cloud Music break through by lifting the exclusive copyright?

Judging from the prospectus, in Netease cloud music revenue, the proportion of membership fees in 2020 is as high as 53.6%, and letting users spend money to listen to music is the main source of profit.

On July 24th this year, according to the anti-monopoly law, the State Administration of Market Supervision ordered Tencent and its affiliated companies to cancel the exclusive music copyright within 30 days, which made the online music market explode.

In other words, this has a greater impact on Tencent Music. Wang Chao, the founder of Yuan Wen think tank, also thinks that this is good for Netease Cloud Music, which has the opportunity to buy some exclusive copyrights that originally belonged to Tencent at a lower price.

"Cancellation of exclusive copyright" is regarded as the lifeline of Netease Cloud Music, but is this really the case?

In 2018, under the impetus of the National Copyright Administration, Tencent Music and Netease Cloud Music transferred their authorized music works to each other, and the two companies only retained 1% of the exclusive music copyright. But this 1% directly affects Jay Chou and Mayday, and many national singers’ songs are controlled by Tencent Music. "Without Jay Chou’s Netease cloud music, there is no soul." A netizen lamented that losing the copyright of some core singers really frustrated Netease Cloud Music.

But in fact, for it, it is not easy to re-light the user’s grayed song list. After all, money still needs to be spent. A record company executive expressed a similar idea about Deep Burning. He guessed that without exclusivity, the copyright fee would be reduced, but their company was unwilling to lower the price. He prefers new forms of binding, such as going deep upstream for content cooperation. This is exactly what Tencent Music is accelerating. In other words, exclusivity has been cancelled, but the threshold of cooperation with music production companies has not disappeared.

At present, the problem of Netease Cloud Music in the music field is not only copyright, but also liquidity efficiency.

The revenue model of online music has not been fully developed. Basically, the platform buys copyright at a high price, and then sells it to users in the form of payment, so as to earn the difference, with a single form.

How much does the copyright price bother Netease Cloud Music? In the earnings conference call of Netease in the first quarter of last year, Ding Lei directly criticized that Netease Cloud Music paid more than 2 to 3 times the reasonable price under the exclusive sales mode of copyright. In the prospectus, from 2018 to 2020, its content service costs were 1.97 billion yuan, 2.85 billion yuan and 4.79 billion yuan respectively, accounting for 171.7%, 123.1% and 97.8% of its revenue respectively.

With such investment, Netease Cloud Music’s "harvest" is that the prospectus shows that in Q1, 2021, the number of monthly users of Netease Cloud Music’s online music service has reached 183 million, but the number of monthly paid users of online music service is only 24 million, which is about one-third of the 60.9 million paid users of Tencent Music in Q1, 2021.

In fact, in 2020, the payment rate of Netease Cloud Music is 8.8%, which is higher than that of Tencent Music’s 7.7%, but compared with the foreign music market, the gap is still very obvious. According to the report data of Guoyuan International Securities in July 2020, the subscription payment rate of Spotify, an overseas music streaming platform, has reached about 50%.

The promotion of the willingness to pay cannot be achieved overnight, and it is worth noting that the premise of its promotion is that the platform needs enough funds to bet on high-quality and even scarce copyright content, so as to attract users to pay. It is not easy for Netease Cloud Music, which has suffered a long-term loss, to spend more money to supplement copyright ammunition.

According to industry-related sources, from 2019 to 2020, Netease Cloud Music was included in the service of paying users of Taobao VIP88 in 88 yuan, and the number of paying users increased from 8.63 million to 16 million. He predicted that Tencent Music would also be connected after Tencent and Ali Eco opened in August. In other words, after Netease Cloud Music Drainage Barrier is broken, its advantages will be further weakened.

Listing is not easy, can social entertainment solve the problem?

Netease Cloud Music is clearly aware that it needs to find a new way out under the pressure that paying users are difficult to break through.

Tencent Music, which achieved profitability earlier, once accounted for more than 70% of the total revenue from social entertainment, relying on the high-margin "online karaoke+live reward" business. Netease Cloud Music is also using social entertainment to "save the country by curve".

Wang Chao explained this realization logic. In terms of consumption power, the member-paid business model is to attract users with low prices, and the business can be at a loss, but it can be realized after the users are introduced. At present, live broadcast realization is a highly efficient business. Since Netease Cloud Music Live Broadcasting Service was launched in the second half of 2018, its revenue has increased from 500 million yuan in 2019 to 2.3 billion yuan in 2020 in just one year.

Doing social entertainment is not difficult for Netease Cloud Music. From the product level, Netease Cloud Music has always been differentiated by song list culture, accurate recommendation mechanism and product tonality. The users account for 89% of the post-90 s young group, which has cultivated a highly sticky community culture.

In recent years, Netease Cloud Music has launched LOOK live broadcast, listening, anchor and other columns around the live broadcast business, which are arranged on various pages. The song room service includes Yuncun KTV, Yuncun singers, parties, etc. At the same time, podcasts are opened to introduce high-quality audio content, and the action is constant. The effect is obvious. In 2018, the revenue of online music services and social entertainment services accounted for 89.4% and 10.6% respectively. By 2020, the revenue of the two major businesses will become 53.6% and 46.4%, and the revenue of the latter will also increase from 120 million to 2.27 billion. The realization channel of Netease Cloud Music is tilting towards "social entertainment", and there is a trend of overweight this year.

Taking advantage of the unique community atmosphere, the latest version of Netease Cloud Music upgrades "attention" to a first-class entrance, so that users can view the trends of friends and musicians more quickly, and the "cloud circle" adds group chat to find the same hobby. The "cloud village" section also integrates Mlog, song room and chat room. But this doesn’t mean that this business can always make great strides.

According to the statistics of Analysys Qian Fan, since September, 2020, the usage time of Netease Cloud Music has shown a downward trend as a whole, and the rich entertainment and social functions are not as good as expected for retaining users.

Whether the appearance of these new functions will affect the original community tonality is not discussed for the time being. An industry insider believes that online music platforms, social media and short video platforms are accessing related content in the market. From a technical point of view, live broadcast rewards such business segments are highly competitive and reproducible, and there are no industry barriers.

In his view, at the same time, the ceiling of such income will not be high. Based on the consumption scene of music socialization, the extended value-added services, the most important of which is to reward the anchor by ordering songs and giving virtual gifts live, which is not much different from other platforms. Tencent Music, Huya and other platforms have occupied most of the market share, and it is very difficult for Netease Cloud Music to "grab food".

And in Wang Chao’s view, at present, around the realization of live broadcast business, it will become rivals with Tik Tok and Aauto Quicker. When Netease Cloud Music attacks the market share of these two platforms, it will not move.

What is the value of original musicians?

Netease Cloud Music also has a trump card-the original music plan.

Netease Cloud Music was previously restricted by copyright, playing independent musicians, with the purpose of differentiating operations and attracting users. More than one person in the industry mentioned that although the platform has a certain traffic tilt to the musicians in the head, after the exclusive copyright is lifted, it will be beneficial to the original musicians at the waist and even at the tail. After all, the deep binding between the platform and the original musicians is equivalent to having scarce content.

The prospectus also mentioned that by December 2020, the number of registered original music of Netease Cloud Music reached 230,000, and the music tracks of registered independent musicians accounted for more than 45% of all music streaming media. Lao Fan, an original musician who has now entered the mainstream vision, became popular in Netease Cloud Music in his early days.

However, from the perspective of revenue, it is difficult to generalize the contribution of original musicians to the platform. Earlier, an independent musician told Deep Burning that Netease Cloud Music had collected a large number of independent musicians’ works for some time, which was a bit like venture capital. "But it’s worth it to hit one." Wang Chao explained that the industry still follows the "28 Law", and only a small number of people can earn money after they become big names.   (two) to provide medical technology and equipment with international leading level;

  (three) can supplement or improve the local medical service capacity, medical technology, funds and medical facilities.

  Article 8 A Sino-foreign joint venture or cooperative medical institution established shall meet the following conditions:

  (1) It must be an independent legal person;

  (two) the total investment shall not be less than 20 million yuan;

  (3) The equity ratio or rights and interests of the Chinese side in a Sino-foreign joint venture or cooperative medical institution shall not be less than 30%;

  (4) The term of joint venture and cooperation shall not exceed 20 years;

  (5) Other conditions stipulated by the health administrative department at or above the provincial level.

  Article 9 In a joint venture or cooperation, if the Chinese side participates in the investment with state-owned assets (including the contribution at a fixed price or as a condition of cooperation), it shall be approved by the corresponding competent department, and the state-owned assets to be invested shall be evaluated by an evaluation agency confirmed by the state-owned assets management department in accordance with the relevant provisions on the evaluation and management of state-owned assets. The evaluation results confirmed by the state-owned assets management department at or above the provincial level can be used as the basis for pricing the state-owned assets to be invested.

  Chapter III Establishment, Approval and Registration

  Article 10 To set up a Sino-foreign joint venture or cooperative medical institution, an application shall be submitted to the municipal health administrative department where the district is located, and the following materials shall be submitted:

  (1) An application for establishing a medical institution;

  (two) the project proposal signed by the legal representatives of the joint venture and the cooperative party and the feasibility study report on the establishment of the Sino-foreign joint venture and cooperative medical institution;

  (3) Registration certificates (photocopies), identity certificates of legal representatives (photocopies) and bank credit certificates of both parties to the joint venture and cooperation;

  (four) the state-owned assets management department’s confirmation document on the evaluation report of the state-owned assets to be invested.

  The municipal health administrative department with districts shall conduct preliminary examination on the materials submitted by the applicant, and put forward preliminary examination opinions according to the regional health planning and medical institution setting planning, and submit them to the local provincial health administrative department for examination together with the application materials, local regional health planning and medical institution setting planning.

  Article 11 The provincial health administrative department shall review the application materials and the preliminary examination opinions of the municipal health administrative department with districts and report them to the Ministry of Health for approval.

  For approval, the provincial health administrative department shall submit the following materials to the Ministry of Health:

  (a) the applicant to set up application materials;

  (2) The Plan for the Establishment of Medical Institutions approved and implemented by the municipal people’s government with the establishment of districts, and the audit opinions of the municipal and provincial health administrative departments with the establishment of districts on whether the proposed Sino-foreign joint venture and cooperative medical institutions conform to the local regional health plan and the plan for the establishment of medical institutions;

  (3) the examination opinions of the provincial health administrative department on the establishment of the Sino-foreign joint venture and cooperative medical institution, including the opinions on the name, site selection, scale (beds, dental chairs), diagnosis and treatment subjects and operating period of the proposed Sino-foreign joint venture and cooperative medical institution;

  (4) Other materials as prescribed by laws, regulations and the Ministry of Health.

  The Ministry of Health shall, within 45 working days from the date of acceptance, make a written decision on approval or disapproval.

  Article 12 An application for the establishment of a Chinese-foreign joint venture or cooperative medical institution of traditional Chinese medicine (including a Chinese-foreign joint venture or cooperative medical institution of integrated traditional Chinese and western medicine and a Chinese-foreign joint venture or cooperative medical institution of ethnic medicine) shall, in accordance with the requirements of Articles 10 and 11 of these Measures, be subject to the preliminary examination of the local municipal health administrative department with districts and the examination of the local provincial health administrative department, and be submitted to state administration of traditional chinese medicine for examination and approval.

  Article 13 After obtaining the permission of the Ministry of Health, the applicant shall apply to the Ministry of Foreign Trade and Economic Cooperation in accordance with relevant laws and regulations, and submit the following materials:

  (1) Setting up application materials and approval documents;

  (2) The contract and articles of association of the Sino-foreign joint venture and cooperative medical institution signed by the legal representatives of the parties or their authorized representatives;

  (3) A list of members of the board of directors of the medical institution to be established as a Sino-foreign joint venture or cooperative medical institution, and a letter of appointment from each party to the joint venture or cooperative medical institution;

  (four) the notice of pre-approval of the organization name issued by the administrative department for industry and commerce;

  (5) Other materials stipulated by laws, regulations and MOFTEC.

  MOFTEC shall, within 45 working days from the date of accepting the application, make a written decision on approval or disapproval; If it is approved, a Certificate of Approval for Foreign-invested Enterprises shall be issued.

  A Chinese-foreign equity joint venture or cooperative medical institution approved for establishment shall, within one month from the date of receiving the Approval Certificate for Foreign-invested Enterprises issued by the Ministry of Foreign Trade and Economic Cooperation, go through the registration formalities with the State Administration for Industry and Commerce by virtue of this certificate.

  Article 14 The application for establishing a Sino-foreign equity joint venture or cooperative medical institution in the central and western regions of China or in the old, less developed, border and poor areas or the scope and content of medical services provided by the Sino-foreign equity joint venture or cooperative medical institution applied for establishment belong to the service field encouraged by the state, and the conditions stipulated in Articles 7 and 8 may be appropriately relaxed.

  Article 15 A Chinese-foreign equity joint venture or cooperative medical institution that is approved to be established shall apply to the health administrative department specified by the local provincial health administrative department for practice registration and obtain the Practice License of Medical Institution according to the procedures and requirements stipulated in the Regulations on the Administration of Medical Institutions and the Detailed Rules for the Implementation of the Regulations on the Administration of Medical Institutions.

  According to the category and scale of Sino-foreign joint venture and cooperative medical institutions, the provincial health administrative department or the municipal health administrative department with districts shall accept the application for practice registration of Sino-foreign joint venture and cooperative medical institutions.

  Article 16 The naming of Sino-foreign joint ventures and cooperative medical institutions shall follow the provisions of the Detailed Rules for the Implementation of the Regulations on the Administration of Medical Institutions issued by the Ministry of Health. The names of Chinese-foreign joint ventures and cooperative medical institutions are composed of local place names, identification names and common names in turn.

  Article 17 Chinese-foreign joint ventures and cooperative medical institutions shall not set up branches.

  Chapter IV Alteration, Extension and Termination

  Article 18 Where an established Sino-foreign joint venture or cooperative medical institution changes its institution scale (beds and dental chairs), diagnosis and treatment subjects, joint venture and cooperation period, it shall go through the corresponding change registration procedures at the original registration authority according to the examination and approval procedures specified in Chapter III of these Measures.

  Changes in the relevant clauses of contracts and articles of association of Chinese-foreign joint ventures and cooperative medical institutions shall be reported to the Ministry of Foreign Trade and Economic Cooperation for approval.

  Article 19 If it is really necessary to extend the joint venture and cooperation period of a Chinese-foreign joint venture or cooperative medical institution after the expiration of 20 years, both parties may apply for an extension of the joint venture and cooperation period, and shall apply for an extension 90 days before the expiration of the joint venture and cooperation period. The application for extension shall be submitted to the Ministry of Health and the Ministry of Foreign Trade and Economic Cooperation for approval after being examined and approved by the provincial health administrative department and the foreign trade administrative department. The examination and approval authority shall, within 45 working days from the date of receiving the application, make a written decision on approval or disapproval.

  Article 20 A Chinese-foreign equity joint venture or cooperative medical institution approved for establishment shall complete the relevant registration procedures within the time limit prescribed by the examination and approval authority; If it fails to be completed within the time limit, the joint venture and cooperation project shall be revoked after approval by the examination and approval authority.

  Chapter V Practice

  Article 21 Chinese-foreign joint ventures and cooperative medical institutions, as independent legal entities, shall be responsible for their own profits and losses, conduct independent accounting and bear civil liabilities independently.

  Article 22 Chinese-foreign joint ventures and cooperative medical institutions shall implement the regulations on the administration of medical institutions and the detailed rules for the implementation of the regulations on the administration of medical institutions on the practice of medical institutions.

  Twenty-third Chinese-foreign joint ventures and cooperative medical institutions must implement the medical technology access standards and clinical diagnosis and treatment technical standards, and abide by the relevant provisions on the clinical application of new technologies, new equipment and large medical equipment.

  Article 24 Medical accidents in Chinese-foreign joint ventures and cooperative medical institutions shall be handled in accordance with relevant state laws and regulations.

  Article 25 The employment of foreign doctors and nurses by Chinese-foreign joint ventures and cooperative medical institutions shall be handled in accordance with the Law of People’s Republic of China (PRC) on Medical Practitioners and the Measures of People’s Republic of China (PRC) for the Administration of Nurses.

  Article 26 In case of major disasters, accidents, epidemics of diseases or other unexpected situations, Chinese-foreign joint ventures and cooperative medical institutions and their health technicians shall obey the assignment of the health administrative department.

  Twenty-seventh Chinese-foreign joint ventures and cooperative medical institutions shall publish their own medical advertisements in accordance with the Advertising Law of People’s Republic of China (PRC) and the Measures for the Administration of Medical Advertisements.

  Twenty-eighth Chinese-foreign joint ventures and cooperative medical institutions shall pay medical fees in accordance with the relevant provisions of the state.

  Article 29 The tax policies of Chinese-foreign joint ventures and cooperative medical institutions shall be implemented in accordance with the relevant provisions of the state.

  Chapter VI Supervision

  Thirtieth local health administrative departments at or above the county level shall be responsible for the daily supervision and management of Sino-foreign joint ventures and cooperative medical institutions within their respective administrative areas.

  The Medical Institution Practice License of Sino-foreign joint venture and cooperative medical institutions shall be verified once a year, and the verification of the Medical Institution Practice License shall be handled by the medical institution practice registration authority.

  Article 31 Chinese-foreign joint ventures and cooperative medical institutions shall, in accordance with the relevant provisions of the state on foreign-invested enterprises, accept the supervision of relevant state departments.

  Thirty-second Chinese-foreign joint ventures and cooperative medical institutions that violate the relevant laws, regulations and rules of the state shall be investigated and dealt with by the relevant competent departments according to law. For Chinese-foreign joint ventures and cooperative medical institutions that violate these measures, the health administrative departments at or above the county level and the foreign trade and economic cooperation departments may punish them according to relevant laws, regulations and rules.

  第三十三条  地方卫生行政部门和地方外经贸行政部门违反本办法规定,擅自批准中外合资、合作医疗机构的设置和变更的,依法追究有关负责人的责任。

  中外各方未经卫生部和外经贸部批准,成立中外合资、合作医疗机构并开展医疗活动或以合同方式经营诊疗项目的,视同非法行医,按《医疗机构管理条例》和《医疗机构管理条例实施细则》及有关规定进行处罚。

  第七章  附则

  第三十四条  香港特别行政区、澳门特别行政区、台湾地区的投资者在大陆投资举办合资、合作医疗机构的,参照本办法执行。

  第三十五条  申请在中国境内设立外商独资医疗机构的,不予以批准。

  第三十六条  各省、自治区、直辖市卫生、外经贸行政部门可依据本办法,结合本地实际制订具体规定。

  第三十七条  本办法由卫生部和外经贸部负责解释。

  第三十八条  本规定自2000年7月1日起实施。

  一九八九年二月十日颁布的卫医字〔89〕第3号文和一九九七年四月三十日颁布的〔1997〕外经贸发第292号文同时废止。

  High-fat diet has adverse effects on intestinal flora.

  Some people say that primitive people did not eat grains, but ate low-carbon water meals. However, archaeological research shows that even in the Stone Age, the energy supply ratio of carbohydrates was around 40%. Moreover, there was no technology to extract oil at that time, so oil was not the food of ancient humans.

  Two or three thousand years ago, oilseeds such as hemp seeds and soybeans were eaten directly, so there were "hemp" and "glutinous rice" (soybeans) in the "five grains" at that time. It was not until after the Song Dynasty that oil became a common food raw material in China.

  Although in recent years, scholars have disputed the question of "what is the best ratio of fat to energy in diet", more than one recent study has found that high-fat diet has adverse effects on intestinal flora.

  Recently, Gut, an authoritative scientific journal, published the new findings of Chinese scientists, which once again proved that eating too much fat increases the harmful flora in the intestine and decreases the beneficial flora.

  In this randomized controlled human dietary intervention study, the researchers recruited 217 healthy people aged 18-35. They have no chronic diseases and no obesity problems. These volunteers ate three different kinds of meals as required for half a year.

  Then, the researchers collected their blood and measured the indexes related to inflammatory reaction, such as IL-1β 、IL-6、IL-8、TNF-α Cytokines, such as PGE2, TXB2, leukotrienes, LTB4, etc. Their feces were collected, and 16S rRNA was sequenced to analyze the flora. During the experiment, all the food and most drinks were provided by the researchers.

  The calorie levels of these three diets are completely consistent, with an average of 2080~2100 kcal, and the dietary fiber intake is also consistent. However, the proportions of the three nutrients are different, and their nutrient energy supply ratios are as follows:

  Type I: the energy supply ratio of fat is 20%, that of carbohydrate is 66%, and that of protein is 14%.

  Type II: The energy supply ratio of fat is 30%, that of carbohydrate is 56%, and that of protein is 14%.

  Type III: Energy supply ratio of fat is 40%, energy supply ratio of carbohydrate is 46%, and energy supply ratio of protein is 14%.

  The fat energy supply ratio of fried instant noodles is about 40%~45%. Cooking with more oil, often eating out, often eating fried dishes, often eating fried cakes and fried dough sticks, and often eating cake biscuits will all increase the fat energy supply ratio.

  The researchers found that after six months, there were no significant changes in the weight, waist circumference, blood sugar and blood lipid of the three groups. However, the intestinal flora has changed significantly.

  First of all, the low-fat diet group with 20% fat has the most diverse fecal flora. High-fat diet reduces the diversity of flora.

  Secondly, the quantitative proportion of flora has changed. High-fat diet increased the number of Alistipes and Bacteroides, and decreased the number of fecal bacteria. Low-fat diet increased bacteria of Blautia and fecal Bacillus. The simple explanation is that the bacteria increased by high-fat diet are mostly varieties that are easy to increase in patients with metabolic diseases such as diabetes; Most of the fungi it reduces are those that are easily lacking in diabetic patients, which produce beneficial components such as butyric acid and reduce intestinal inflammatory response.

  The third finding is that the short-chain fatty acids in the feces of subjects in the 40% fat diet group are significantly lower than those in the other two groups. This is not good news, because butyric acid produced by large intestine fermentation is beneficial to control blood lipids, and recent research has just found that it is beneficial to reduce the risk of diabetes.

  The fourth finding is that in the low-fat diet group, the contents of cresol and indole in feces are relatively low, and these components are less, indicating that it is far from various chronic diseases. Conversely, there are more arachidonic acid and lipopolysaccharide in the feces of high-fat diet groups, and more of these components represent higher risk of inflammatory reaction.

  The analysis of blood samples shows that many proinflammatory factors in plasma of high-fat diet group are relatively high. In the 40% fat group, the content of TXB2 was significantly higher than that in the 20% low-fat group, and the content of C-reactive protein was significantly higher than that in the middle-fat diet group with 30% fat. Conversely, in the low-fat diet group with 20% fat, the contents of LTB4 and PGE2 were lower than those in the other two groups. In a word, a high-fat diet will obviously promote the inflammatory reaction.

  This means that those who are prone to acne and inflammation should avoid eating high-fat meals.

  Too much fat can lead to obesity and various chronic diseases.

  In fact, the results of this study are not too surprising.

  In fact, as long as Chinese people recall the past a little, they have to admit that in the past, when food was the mainstay and they could not eat much oil, people were very thin, and chronic diseases such as diabetes, fatty liver and coronary heart disease were rare, and intestinal cancer was also rare. With the development of economy, the only thing that keeps pace with the income level is fat. At the same time, obesity and various chronic diseases have increased.   In this study, although the ratio of fat to total calories is different among the three groups, the total calories and total dietary fiber intake are strictly consistent.
