Professor Wang gave today’s lecture from three aspects: the overview of TCM thinking, the thinking of clinical diagnosis and treatment of TCM, and the application of gynecological TCM clinical thinking. Professor Wang summed up: the traditional Chinese medicine thinking of gynecological diagnosis and treatment, that is, the correspondence between man and nature and the unity of form and spirit; Chinese and western convergence, multi-disciplinary; Combination of symptoms and diseases, three factors are suitable; Internal and external treatment, partial whole; Acupuncture and drug acupuncture, multi-method application; Prevention before illness means prevention of illness and recovery after illness.

Professor Zhang Yi, Beijing Hospital, The Methods and Advantages and Disadvantages of Vaginal Hysterectomy

In recent years, with the development of minimally invasive technology, vaginal surgery has developed rapidly in obstetrics and gynecology because of its advantages of safety, minimally invasive and rapid recovery. Professor Zhang said that hysterectomy is the most common operation in gynecology. Whether it is abdominal hysterectomy, vaginal hysterectomy or laparoscopic hysterectomy, it has advantages and disadvantages. Advocating individualized treatment can benefit patients more. The advantages of vaginal hysterectomy are minimally invasive operation, minimal peritoneal opening, avoiding infection of abdominal incision and quick postoperative recovery. However, it also has a small surgical field of vision and high technical requirements of the operator, which requires the operator to reasonably grasp the indications and contraindications of the operation.

Professor xuanwu hospital Wang Shijun of Capital Medical University, "How do gynecologists face urinary system injuries? 》

The topic brought by Professor Wang Shijun is "How do gynecologists face urinary system injuries?" . Professor Wang focused on the treatment of four kinds of ureteral injuries during operation: ① suspected thermal injury: preventive DJ tube implantation; ② Low injury: ureteral bladder implantation; ③ High injury: end-to-end anastomosis and wall repair; ④ Segmental injury: bladder and kidney are free, bladder flap and ureter anastomosis and even kidney font transplantation. Finally, Professor Wang also told us about the treatment of bladder injury, which needs suture and repair during operation. Postoperative repair after 3 months, including laparoscopic or laparotomy, transvaginal, vaginal-abdominal combination, intravesical laparoscopic repair.

Professor Cui Heng, Peking University People’s Hospital, Surgical Treatment Strategies for Ovarian Cancer

Professor Cui explained in detail the diagnosis and treatment status of ovarian cancer, the strategy of surgical treatment, how to improve the quality and effect of surgical treatment, etc. Generally speaking, the core of ovarian cancer surgery is to completely reduce the tumor, and the reduction of RO is the basis for ovarian cancer to obtain good curative effect. Accurate preoperative evaluation is the basis of appropriate decision-making, and attention should be paid to preoperative evaluation of lymph nodes; The indications of neoadjuvant chemotherapy should not be expanded unprincipled; Lymphadenectomy is of great value in the surgical treatment of ovarian cancer, including diagnostic value: accurate evaluation of lymph node metastasis and accurate staging of tumor. Therapeutic value: resection of metastatic or possibly metastatic lymph nodes, complete tumor reduction, guiding postoperative treatment to prevent recurrence, improving prognosis and prolonging survival time. No lymphadenectomy was found in patients with stage IIIc and IV. If the initial operation is not removed, it should also be removed in intermittent surgery.

Professor Hao Zengping from Friendship Hospital affiliated to Capital Medical University, Vaginal Microecology and HPV Infection

Female lower reproductive tract is an open channel, which is an important microecological area in human body. Vaginal microecosystem is not only complex, but also very sensitive, and it is easy to change when it is influenced by endogenous and exogenous factors. Professor Hao explained the vaginal microecology and HPV infection respectively, focusing on the correlation between vaginal microecology and HPV infection. Professor Hao pointed out that a British study analyzed the relationship between vaginal microecology and cervical lesions in 169 women, and found that the low-grade intraepithelial neoplasia of CST4 patients was twice as high as that of normal people. The development of high-grade intraepithelial neoplasia is three times that of normal people. Finally, Professor Hao summarized the mechanism of vaginal microecology and cervical lesions: ① Pathogenic bacteria can increase the pH value of vaginal microenvironment, prolong the metaplasia period of squamous epithelium and promote epithelial cell dysplasia. ② At the same time, harmful metabolites such as propionic acid and butyric acid are produced to cause epithelial cell damage. ③ The pathological process is more conducive to the infection of HPV and other therapeutic factors, and plays a synergistic role in the occurrence and development of CIN and cervical cancer.

Professor Huang Jinchang from the Third Affiliated Hospital of Beijing University of Chinese Medicine introduced the highlights of TCM treatment of gynecological tumors.

Professor Huang Jinchang gave a wonderful report on the prevention of rectovaginal fistula caused by chemotherapy of ovarian cancer with traditional Chinese medicine, acupuncture to improve the efficiency of chemotherapy in patients with ovarian cancer, external treatment of traditional Chinese medicine to treat incomplete intestinal obstruction caused by radiotherapy of cervical cancer, moxibustion to support the smooth progress of chemotherapy, moxibustion to treat ovarian cancer ascites, and guiding the diet of patients with ovarian cancer according to pathological types. When introducing the treatment of ovarian cancer ascites by moxibustion, Professor Huang talked about the treatment of malignant ascites: ① Oral administration of traditional Chinese medicine-based on the situation of primary organ tumors and whether portal hypertension, hypoproteinemia and renal insufficiency occur. ② Umbilical moxibustion —— Li Zhao San (Radix Astragali, Herba Asari, Zanthoxylum, Solanum nigrum, Ramulus Cinnamomi, and Radix Kansui) is ground into fine powder, applied to Shenque point, and moxibustion at Shenque point is conducted for more than 1 hour every day, and the first time is more than 3 hours. At present, all of them use Li Zhao Ai, which is specially used to treat hydrothorax and ascites. ③ Acupuncture-treatment based on syndrome differentiation.

Professor Wang Dongmei, Affiliated Hospital of Shandong University of Traditional Chinese Medicine, Prevention of Endometrial Cancer

At present, the etiology of endometrial cancer is not very clear. In the ACOG Practice Guide published by the American Obstetricians and Gynecologists Association (ACOG) in 2015, it is pointed out that the long-term effect of estrogen without progesterone antagonism (whether endogenous or exogenous) is related to the pathogenesis of type I endometrial cancer. Compared with patients who have not received hormone replacement therapy, patients who insist on using progesterone have a lower risk of endometrial cancer. The Expert Consensus on Screening and Early Diagnosis of Endometrial Cancer does not recommend routine screening for women with average risk and asymptomatic endometrial cancer, but recommends screening for women with high risk of endometrial cancer. At present, it is recommended that high-risk groups be screened every year. In addition, Professor Wang also mentioned that Chinese medicine has its own characteristics in the prevention and treatment of tumors, which contains rich preventive medicine ideas. Emphasize the three elements of preventing disease: preventing disease before disease; Prevention of disease and change; Recovery after illness.

Professor Xu Chunlin, Second Affiliated Hospital of Hebei Medical University, "Postmenopausal Genitourinary Syndrome GSM"

Professor Xu Chunlin made a detailed interpretation of the definition, pathophysiology and clinical manifestations, diagnosis and treatment of GSM respectively. Professor Xu talked about five treatments for GSM, one of which is vaginal laser therapy: ① The existing data show that vaginal laser can induce morphological changes of vaginal tissue (increase the thickness of vaginal epithelium, increase the content of vaginal glycogen and collagen, and promote vascular reconstruction), and relieve vaginal dryness and sexual pain related to GSM. ② Micro-ablation CO2 laser and non-ablation bait laser(VEL). ③ For patients with mammary gland or high risk factors, non-hormone therapy should be the first choice to relieve symptoms. ④ Vaginal laser lacks RCTS and long-term safety research, but after evaluating the potential advantages and disadvantages, the need and cost of continuing treatment, patients who prefer non-hormonal treatment are preferred. Finally, Professor Xu Chunlin summarized this topic.

Professor Song Dianrong, Second Affiliated Hospital of Tianjin University of Traditional Chinese Medicine, The Application and Advantages of Integrated Traditional Chinese and Western Medicine in the Treatment of Gynecological Diseases

Professor Song shared the topic from five aspects: the background and advantages of Chinese medicine and western medicine, the complementary development and advantages of Chinese medicine and western medicine, the understanding of gynecological diseases between Chinese medicine and western medicine, the idea of treating gynecological diseases with integrated Chinese and western medicine, and typical case analysis. Among them, Professor Song shared five forms of diagnosis and treatment of diseases by combining traditional Chinese and western medicine: ① Western medicine diagnosis+traditional Chinese medicine treatment: if western medicine diagnosed menopausal syndrome, patients refused to accept hormone replacement therapy, and were treated with traditional Chinese medicine such as Zuo Gui Dan, You Gui Dan, and two to nine; ② Diagnosis and treatment are carried out in two ways, that is, dialectical western medicine plus dialectical traditional Chinese medicine: for example, in the acute stage of pelvic inflammatory disease, heat and toxicity are flourishing, and antibiotics are combined with traditional Chinese medicine (Wuwei Disinfection Drink) for clearing away heat and toxic materials, promoting diuresis and expelling pus; ③ Traditional Chinese medicine as a supplement to western medicine: for example, surgical treatment and postoperative treatment of ovarian cancer, while chemotherapy is supplemented with traditional Chinese medicine to strengthen the body resistance and reduce the side effects of chemotherapy; ④ Specific treatment of western medicine plus individual dialectical treatment of Chinese medicine: for example, in the treatment of infertility, western medicine promotes ovulation and corpus luteum support, and at the same time, different Chinese medicines are used in different periods of menstruation to promote follicular development, grow endometrium and help pregnancy with food and clothing; ⑤ Accept the diagnosis and prescription treatment of both Chinese medicine and western medicine at the same time: if the patient sees both Chinese medicine and western medicine, and takes both Chinese medicine and western medicine, there is no connection between them (not advocating).

Professor Zhang Jun shared the multi-disciplinary hierarchical management of cardiovascular diseases during pregnancy. Professor Zhang explained the screening, evaluation, classification and risk prediction of cardiovascular diseases during pregnancy with a large number of clinical cases and expert consensus system. It is pointed out that multidisciplinary cooperation (MDT) is the key to ensure the complicated and critical pregnant women get more timely and reasonable treatment, and it is also the key to improve the maternal and infant outcomes. MDT team comprehensively covers maternal management, hierarchical evaluation and multidisciplinary consultation in outpatient MDT; In-depth management of MDT in wards, cutting-edge surgery, treatment and innovation involving disciplines; Critical MDT opens a green channel to race against life.

Management of Postpartum Hemorrhage by Professor Jiang Rongzhen from Shanghai Sixth People’s Hospital

Postpartum hemorrhage is an important cause of maternal death. What Professor Jiang Rongzhen shared with you is the liquid management of postpartum hemorrhage. Professor Jiang said that to prevent postpartum hemorrhage, we should be familiar with the basic diseases of pregnant women before delivery, pay close attention to placental factors, and use drugs scientifically and effectively during and after delivery. Fluid management before bleeding (the first stage of labor) includes identifying high-risk factors and preventing postpartum hemorrhage; The second stage of labor includes avoiding uterine contraction and opening venous access; Accurately evaluate the blood loss when bleeding, emphasize the construction of postpartum hemorrhage treatment team, and take measures to deal with postpartum hemorrhage at any time. Give fluid resuscitation to the parturient in case of severe bleeding.

Professor Liu Hongmei, Rehabilitation Hospital Affiliated to National Rehabilitation Aids Research Center, Diagnostic and Therapeutic Strategies of Fetal Water Disease in Traditional Chinese Medicine.

Fetal water is the ocean that breeds the fetus. Professor Liu Hongmei will give a lecture today on the diagnosis and treatment strategy of fetal water disease in traditional Chinese medicine. Liu Jiaoshou introduced that any amniotic fluid volume exceeding 2000ml in any period of pregnancy is called "polyhydramnios", and Chinese medicine is called "full fetus" and "full fetal edema"; The amount of amniotic fluid in the third trimester of pregnancy is less than 300ml, which is called "oligohydramnios", which is called "fetal wilting is not long" in Chinese medicine. Liu Jiaoshou summarized the TCM treatment strategies of fetal water disease: differentiating zang-fu organs, treating spleen at early stage, treating kidney at middle stage and treating lung at late stage, and treating the symptoms based on clinical experience, correctly diagnosing the cause and treating the cause.

Professor Li Daocheng, First Affiliated Hospital of Guangzhou Hospital of Traditional Chinese Medicine, Prevention and Treatment of Postpartum Urinary Retention with Traditional Chinese Medicine

Postpartum urinary retention refers to the disorder of micturition in parturient, which mostly occurs 6-8 hours after delivery. It is one of the common complications after delivery, which will cause maternal anxiety and urinary tract infection, lead to postpartum hemorrhage and reproduction, lead to bladder rupture and serious complications of renal failure. Professor Li said that the prevention and treatment methods of western medicine include psychological counseling and health education, scientific management of labor, pelvic floor muscle training and low-frequency electrical stimulation. The prevention and treatment methods of traditional Chinese medicine include external use and internal treatment. External use includes acupuncture therapy, moxibustion therapy, acupoint application, acupoint pressing, acupoint injection and auricular point pressing, etc. Internal treatment mainly includes oral administration of traditional Chinese medicine, which is treated dialectically according to the clinical manifestations of patients.

Professor Wang Zhe, Affiliated Hospital of Shandong University of Traditional Chinese Medicine, "Treatment of Fetal Dyskinesia in the Middle and Late Pregnancy with Integrated Traditional Chinese and Western Medicine"

Threatened abortion refers to backache, abdominal pain, lower abdomen falling during pregnancy, or accompanied by a small amount of vaginal bleeding, which is called "fetal movement uneasiness" in Chinese medicine. Professor Wang Zhe introduced that the etiological mechanism of fetal movement in TCM includes kidney deficiency, blood heat, qi and blood weakness and blood deficiency; The diagnosis should comprehensively consider the medical history, clinical manifestations and auxiliary examination results of pregnant women, and treat them according to their clinical manifestations. Later, Professor Wang shared with you the experience of treating late threatened abortion with SCH and cervical incompetence with integrated traditional Chinese and western medicine, pointing out that the treatment of late pregnancy with fetal movement disorder was based on dialectical treatment of traditional Chinese medicine and symptomatic treatment of western medicine, which realized both the symptoms and root causes and effectively improved the clinical efficacy.

Professor Cong Huifang, Second Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine, Prevention and Treatment Strategies of Postpartum Depression in Traditional Chinese Medicine

Professor Cong Huifang took the incident of a 30-year-old woman jumping off a building with her daughter in Xiangtan on April 22 as the starting point of this lecture. Professor Cong introduced that postpartum depression disorder is referred to as "postpartum depression". At present, the incidence of postpartum depression is internationally recognized as 10-15%, which is three times that of the general population in the first five weeks after delivery, and gradually decreases in June after delivery, close to the general population. Postpartum depression not only harms the mother herself, but also harms the child’s health. The risk factors of postpartum depression include past mental history, life stress time, marital quality and obstetric factors. Professor Cong introduced the screening and diagnosis process of postpartum depression in detail. He also said that due to postpartum hemorrhage and blood stasis, it is necessary to make a clear diagnosis, judge the severity, treat patients dialectically according to their specific manifestations, and establish a good doctor-patient relationship.

Professor Bai Guiqin, First Affiliated Hospital of Xi ‘an Jiaotong University, Clinical Application and Progress of Key Techniques for Mother-to-child Blocking of Hepatitis B

China is a high incidence area of hepatitis B. Professor Bai Guiqin said that the risk factors of mother-to-child transmission of HBV include high HBVDNA load, HBeAg positive, drug-resistant virus and HBV gene. HBV screening should be performed before pregnancy. With regard to the treatment and management of pregnancy, all pregnant women who have first-time check-up in outpatient clinics need to check hepatitis B as required, and distinguish whether pregnant women are carriers of hepatitis B immune tolerance or active hepatitis B, so as to lay a solid foundation for hierarchical management of patients. Chronic hepatitis B carriers are given individualized management according to HBVDNA content. Professor Bai said that combined immunization has been proved to be the most effective measure to prevent mother-to-child transmission of HBV.

Professor Ning Yan of Shenzhen Maternal and Child Health Hospital "On the Clinical Application of Classic Prescriptions in Postpartum Diseases"

A pregnant woman’s illness related to childbirth within one month or one hundred days after delivery is called "postpartum disease". Professor Ning Yan’s lecture is about the clinical application of classic prescriptions in postpartum diseases. Professor Ning said that the causes of postpartum complications are probably nothing more than sudden deficiency of qi and blood, impaired vitality, fluid deficiency and less blood. Professor Ning introduced the clinical application of classic prescriptions in postpartum complications with clinical cases, and pointed out that in the clinical application of classic prescriptions, based on the principle of not staying in postpartum and not forgetting postpartum, the methods of strengthening the body resistance, eliminating pathogenic factors and removing blood stasis should be used reasonably, and dialectical treatment should be carried out according to the condition. Postpartum critical illness should be treated with combination of traditional Chinese and western medicine.

Management Strategy of Early Pregnancy Complicated with Hypothyroidism by Professor Li Li of Guangzhou Women and Children Medical Center

Professor Li Li gave a lecture on the management strategy of hypothyroidism in early pregnancy. Clinically, TSH exceeds the upper limit of reference value and FT4 is below the normal reference range, which is diagnosed as clinical hypothyroidism in pregnancy. A large number of studies have proved that there is a correlation between hypothyroidism and poor pregnancy outcome; Hypothyroidism of pregnancy and TPOAb+ have a combined effect on abortion; Untreated hypothyroidism does not increase the IQ risk of offspring. In 2017, the guidelines of the American Thyroid Association pointed out that hypothyroid women receiving LT4 treatment should independently increase the LT4 dose by 20-30% if pregnancy is suspected or confirmed, and immediately notify their responsible physicians for immediate testing and further evaluation.
